Before you arrive for your first appointment, though, we want to make sure you know all the basic information about what you can expect. On the following page, you will learn more about your initial visit, our available payment options, and any new patient paperwork that is required. Your First Visit
When calling to schedule your first appointment, we will send digital paperwork for you to complete before your arrival. On the day of your visit, you will be escorted to a treatment room and have a full set of X-rays taken before one of our dentists completes a thorough evaluation. After reviewing the images and compiling their information, Dr. Daniel Eley or Dr. Joel Eley will craft a treatment plan and determine which type of cleaning is required for your visit. If enough time is available, we will complete a professional teeth cleaning during the same appointment.
Patient Forms
After you schedule your first appointment with us and before you arrive to our office, you’ll receive a link to digitally fill out your new patient forms. Or, if you prefer, you can arrive 15 minutes before your scheduled appointment to complete your paperwork electronically in-office on one of our tablets.
Dental Insurance is Welcome!
Eley Family Dentistry is an insurance-friendly dental practice that is in-network with most PPO plans. Some of these include Delta Dental, Cigna, MetLife, and more. Our team will go over the details of your policy when you call to schedule an appointment and determine how we can maximize your available benefits. This ensures you do not miss out on valuable savings while working toward a healthier smile. If you have questions about your plan, do not hesitate to call our office.
In-House Membership Plan
If you are without private dental insurance and prefer not to finance your treatment, you can enroll in our in-house membership plan. For one low annual fee, you can receive complimentary exams and cleanings, as well as discounts on all other services.
